Has anyone had any success with the metatags/friendly SEFurls/modrewrite in Joomla?

I've been told my an SEO company that my domain has 6 versions of my home page: domain.com/domain.com/index.php, domain.com/index/stuff/stuff/etc, www.domain.com, etc.

Is there a way to fix this? Create new titles for my products pages? For example, instead of my homepage, domain.com, showing "Home" as the top of the window, I'd like to change it to reflect my website products.
