Part of the S5 No moomenu is not working with Joomla SEO setting

For the items on first level, the url is shown proper like www.website.com/joomla/first-section.html

However the sub menu  ( the second level) is not shown properly.

It show something like www.website.com/joomla/sub-page.html
instead of   www.website.com/joomla/first-section/sub-page.html

The SEO settings is working proper for all mod_mainmenu  module but not in S5-no-moomenu

Is there any patch for this?

I am running Joomla 1.5.3 on CentOS 5.1

I have setup the S5 No moomenu with the Forever Aces template.

In Joomla thethree  SEO Settings is set to "Yes"
Search Engine Friendly URLs   
Use Apache mod_rewrite     
Add suffix to URLs 

Legacy mode is enable.
