I've seen multiple post about this error and each person has his/her own resolution to it but I've not seen those resolutions posted. I just started to get the error 500 - JHtml icon not found. when I try to go to certain articles thru a direct menu link on my website. So if I have a submenu item linked to a single article, when I try to get to the article thru the sub menu link I get the error 500 - JHtml icon not found. on the page that opens. However, I can get to the article if I go to the category menu level then just click on the topic links. I'm using lastest Maxed Mag version. Vertex 6. That sounds like an issue with your SEF settings. In the Global Configuration have you tried disabling SEF URLs and URL Rewriting?
