I am guessing someone bothered to make a bot that can read the captcha image. Either that or they guess the code with knowledge ahead of time of what the possible values are. I mean its a 1 in 5 chance at that point or something?

Make sure you are running the latest version of S5 Register and have captcha enabled. The captcha works pretty well at preventing this.
