I am having an issue with the form maker component and the zoka template.  It displays a blank page in the browser on any pages with any form created by the form maker extension.

I contacted Web Dorado(Developers of form maker)  to help out and this is their reply below:

Please be informed, that your website template does not allow adding scripts to its header and that is why forms aren't loading. Could you please contact your template developers and ask them to take a look?

How do I allow the scripts to be added to the header?

Hello,

A Template doesn't block adding "scripts" in the header nor would that even be possible. You can literally open up the index.php file and add anything in the header that you wanted. You would need to contact your 3rd party developer as we do not support or debug non Shape5 products.

Funny thing is I can use other templates with the same form, and they work.  I have other templates like  swapps that are also causing issues. Am I missing a setting in the template area.

Hello,

This means there is a script conflict between your 3rd party extension and one of the scripts used in the Framework or Extensions. This could be Multibox, S5 Box, Page Scroll etc which all use javascript files. I would suggest disabling any scripts in the "Javascripts" tab of the Template Manager to see if this helps the issue.

Beez5 is just a simple HTML page without any style sheets or scripts.

It was the S5 contact extention.  I just disabled on the form maker pages and everything works fine.
