I am trying to use the Contact plugin as it's own page on my website.  However, when trying to assign the page to display the widget on, the "Contact" page I created is not an option in the drop-down list (see options given below).   How do I specifically assigned the widget to just one page if it is not in the list?  My site is http://www.reconciliationmin.org

That is a feature of Widget Context (https://wordpress.org/plugins/widget-context/), which is what is used on all the demos to allow different layouts per page.

The "Target by URL" box is where you designate specific pages or sections of the site where the widget will show/hide. 

If the permalink for your contact page is "contact-us", then enter "*contact-us*" in the target box, and you should get the results you want.
