I'm going through the process of uploading a theme to my hosting site.  I followed along with the video tutorial and I'm at the part concerning the phpMyAdmin section.  It's asking me to create a database.  But I don't see an option for that.  Is there something else I should have downloaded?

The tutorial video is for installing a Site Shaper, if you are just installing the theme, you can just use the "Upload" option for install on the "Appearance -> Themes" admin page.

If you actually want to install the shaper, the Wordpress FAQ (http://www.shape5.com/component/option,com_smf/Itemid,75/topic,23923.0/) has a very easy tutorial for how to install a Site Shaper.  I suggest trying to use that; if you have difficulty, feel free to post more specific questions so I can know what you actually need help with.

I am using the free Forte theme.  Should I be using the site shaper in conjuction with another theme?  I only loaded the site shaper to root folder.

A shaper is a complete WP install, including the theme and all demo content.  As with any WP install, it assumes that you have already created a database for WP -  if you do not already have a database, you will need to create one.  Creating a database can be very different from server to server, so you'll want to check with your hosting provider to see how that needs to be done on your server.

The easier way would be to use an auto-installer which most host have to install a basic version of WP, then follow the instructions on the Wordpress FAQ (http://www.shape5.com/component/option,com_smf/Itemid,75/topic,23923.0/) for installing the shaper on an existing site.
