I have an implementation of User Choice at http://clarionupgrade.tnhillbillie.net/ -- and I've migrated content from a 1.0 site.

For #3 -- Whenever I click on any link on my test site (clarionupgrade.tnhillbillie.net), the string index.php appears as part of the link's path. I recalled having read something about SEF URL's a while back, so I played with that setting in the Global Configuration. That was it.
