From looking at the Javascript console in
Firebug, it looks like there's an error in the code for the form. There is also an error from the map that says you don't have a valid API key installed, so it's possible that the maps error is causing a conflict; it may be solved by making sure your API key is correct in the map widget.

I created a api key for the map widget and the problem still happens. Like i said above, the form works very fine in other themes, so i guess there is something make conflict with this theme, do you have any other idea?
Also I contacted to the support staff of "Caldera" and they said me that going to see if they can do something and that feel me free to give you his email
